* Fixed bug/issue with magic_quotes_gpc
  - Description
  As in previous fix, however also updated the general survey section so that any updates or new
  surveys, questions, answers etc. also checks for magic_quotes_gpc. Previous fix only applied
  to the public survey script (ie: index.php)

* Fixed bug/issue with dataentry screen where any html tags would muck up the javascript alert box
  - Description
  The popup help buttons in the dataentry screen would be broken if the help contained any html
  code. Added code to strip html tags from text before placing in alert box!

* Increased size of "Group Title" field.
  - Description
  It was only 50 chars long, but this was way too small. Now it's 100.

* Added Group Descriptions
  - Description
  Sometimes a group of questions can be difficult to interpret without an explanation preceding them.
  A description field has been added to the 'groups' section. When the public survey is being
  completed, and there is a description in the survey, this description will be displayed before
  any of the questions.
  This will hopefully provide the first step in providing "conditional" groups for a later release.

* Added "Copy Question" facility
  - Description
  Don't you hate it when you've got 10 questions in a row, and they're all almost exactly the
  same, but you've got to type each one out, then add all the pre-defined answers and so on?
  This feature allows you to copy a question, and (optionally) all its associated answers into
  a new question, thus saving you lots of time!

//** Changes for Release 0.92

**** NOTE *****
See lower section (changes for release 0.91) if you are upgrading from 0.90
***************
* Fixed bug/issue with magic_quotes_gpc
  - Description
  Script now automatically detects whether magic_quotes_gpc is on or off and uses addcslashes
  if needed, but not if not needed. This stops a submit bug where cslashes are added and magic_quotes_gpc
  is on.

* Fixed bug/issue with timeouts and sending invitations
  - Description
  New entry in config.php allows you to set a maximum number of emails to be sent at once.
  Previously if you tried to send out too many emails, your server MAY have timed out
  during the process. While this usually did not cause a huge problem (apart from the last
  attempted email not being updated to indicate it had been sent), it means if you typed
  in your own text in the message body of the email, you'd have to do it again to send the
  rest.
  - Resolution
  Set the maxemails setting in config.sys to the most suitable number. Most servers with their
  own SMTP server on the same computer/LAN will be able to do quite a few at once. The default
  is 100. On my home computer, which has to connect to my ISP SMTP Server on a different computer
  I have to limit it to 15

* Added "Printable Version"
  - Description
  This is my somewhat flawed attempt to deal with people who want a printed copy of the survey
  to fax to participants who cannot do the online version. It works fairly well, producing
  a web page that, when printed out, is fairly neat and could be used quite succesfully.
  You can access the "printable version" from the "browse" screen - once a survey is activated.
  Although the "printable version" looks like a web form, there is no submit button. Use
  the "dataentry" feature for online dataentry.
  Related to this, there is an entry in config.php for a "reply fax number", which gets printed
  at the bottom of the printable form telling people to fax the completed survey back to that
  number.

* Added additional question type "List with comment"
  - Description
  This allows a set of radio buttons with a comment, so you can ask a question like:
  "In the following list of options, choose the one you'd like and explain your choice"

* A bit more 'prettying up' of the public survey interface.

//** Changes for Release 0.91

**** NOTE *****
To add additional fields for release 0.91 run the file "upgrade091.php"
***************

* Fixed bug/issue with import of exported survey question files.
  - Description
  In previous release exported survey files that contained db entries with a hard or soft return
  would cause an import of the same file to fail. This was due to the 'read' method of the import
  script, which reads one line at a time, however a soft or hard return would prematurely end
  the line and cause the survey not to be imported.
  - Resolution
  Export function fixed to replace any hard or soft returns

* Fixed bug/issue with htaccess and htpasswd in windows/linux
  - Description
  In previous release script was not always able to succesfully determine if windows or linux was
  being used. Also, adding security would sometimes file to create a htpasswd file, but would
  succesfully create htaccess, thus restricting access to the directory, but providing no default
  username and password.
  - Resolution
  Better way of determining OS. Also, error checking when installing security so it deletes the htaccess
  file if the htpasswd file does not get created.

* Fixed bug/issue with browse display of results
  - Description
  Various problems were occurring with the new survey question types resulting in them not displaying
  properly in the 'browse' screen.
  - Resolution
  Bug fixed

* Fixed bug with export of results containing hard or soft returns
  - Description
  CSV files really don't like hard or soft returns. They consider them to be the end of a row and as
  far as I can see there is no way of allowing a hard or soft return to get passed through within a value.
  - Resolution
  Hard and soft returns removed during export and replaced with spaces.

* Fixed bug where "add answer" was still an option for multiple choice type questions even after the
  survey had been added.

* Added additional logic checking for database tables.
  - Description
  Previously script just checked to see if the database existed, but couldn't cope if the database
  existed but didn't have any tables. This could be a problem where a user doesn't have rights on
  a MySQL database to create a db, but has had an empty db created for them. Now the script will
  create any missing tables before going to normal admin mode.

* Added new survey question types:
  - Description
  Added "multiple choice with comment", "array with 5 point choice", "array with 10 point choice" and
  "array with yes/no/uncertain"

* Added new export option:
  - Description
  Added option to export to a csv file.

* Added some logic checking to users table
  - Description
  If user in table doesn't exist in htpasswd file, user is removed!

* Added expiry date to surveys
  - Description
  This additional field in the survey table allows you to set an expiry date for your surveys.
  Once the expiry date has passed nobody can use the 'public' data entry script (however the
  standard dataentry script remains available)

* Added admin email to surveys
  - Description
  This additional field allows for emailing of tokens to use the admin name and email fields
  as the 'reply-to' and sender name for emailed invitations to participate in the survey.

* Split config.php into two files
  - Description
  A lot of things that shouldn't be modified by users were in the config.php file in the 0.9 release
  and these have been moved to a new file called "common.php" in 0.91. Hopefully this will stop
  configuration looking too frightening and reduce the chance for muckups.

* Various 'prettying up'
  - Description
  Added preset styles to buttons and fields. Styles can be adjusted to taste in the config.php file.
  Replaced hyperlinked text with buttons in some places. Import of questions has been given a
  standardised page with header etc. Put a logo in footer that links to sourceforge page.
